Your patient portal

Get into the portal in minutes

The patient portal is live, and there is no password to create — sign in with a one-time code sent to your email. New clients receive a secure onboarding link from the practice that sets up their profile. Once you are in, everything about your care is in one secure place.

Guided onboarding

New

New clients walk through a guided intake: profile, contact and emergency information, insurance or self-pay, and the demographics your provider needs — collected once, kept up to date.

Smart home dashboard

New

Your portal home shows exactly what needs attention: forms to sign, upcoming appointments, new screeners, and unread messages. Nothing gets missed.

Appointments

New

View your upcoming and past appointments, session types and scheduling details — and request a new visit right in the portal, choosing from your provider's real available times.

Pre-session check-in

Before each appointment, rate your week, flag topics to discuss, and note any concerns. Nicholas sees your responses before the session starts — so you never have to re-explain everything from scratch.

Medications & consent signatures

View your current medication list with dosing details. When a new medication is prescribed, review the education materials and sign your consent directly in the portal.

Clinical notes & visit summaries

Access progress notes, psychiatric evaluations, and after-visit summaries written for you — key points from each session, medication changes, and next steps.

Lab results

Lab results ordered or shared by your provider appear in the portal. No more waiting for a phone call.

Wellness snapshot (AI-assisted)

New

Open the Progress tab and choose Wellness snapshot: a plain-language overview of how your assessment scores, mood and symptom logs are trending between appointments. Your provider shares one with you when it is ready, and you can ask for a fresh one yourself — that button is governed by AI assistance in Settings → AI & privacy.

Documents — upload & download

New

Upload your own health documents (prior records, insurance cards, referral letters) and download any files Nicholas has shared with you.

Wellness tools

Daily log

New

Rate your mood 1–10 plus anxiety, energy, sleep and concentration in seconds, under Assessments → Daily log, and see the patterns over time under Progress → Trends — which you can review with Nicholas at sessions.

Guided wellness journal

New

A private journal with guided prompts. Opt in to AI reflections that respond to your entries, or keep everything to yourself.

Mental-health screenings

New

Complete standardized screeners assigned by Nicholas — PHQ-9, GAD-7, PCL-5, ASRS, MDQ, ISI, and more. Results are scored automatically and feed directly into your chart.

Your daily log reaches your chart

New

What you record in the Daily log is visible to your provider in your chart, so a week of poor sleep is on the screen at your next appointment without you having to remember it.

Personal safety plan

New

A personalized crisis plan — warning signs, coping strategies, supportive contacts, and professional resources — that you build and update with Nicholas.

Therapeutic worksheets

CBT thought records, mindfulness activities, and other exercises assigned by Nicholas, completed and submitted directly through the portal.

Assigned resources

Your provider can share handouts, worksheets, and links directly to your portal — review them between sessions, and mark each one complete as you go.

Communication & requests

Secure messaging

Message Nicholas between sessions — HIPAA-compliant, private, and never leaving the secure system. For non-urgent questions; emergencies should always use 911 or 988.

Join your telepsychiatry sessions directly from the portal — your secure video room is one click away, no downloads required.

Medication refill requests

Request a refill without a phone call. Submit through the portal and track its status; Nicholas receives it immediately.

Release of Information

Authorize sharing of your records with other providers, insurers, or family members — digitally, with a full signature audit trail.

Assigned form completion

When Nicholas sends you a form or questionnaire, it appears in your portal — complete and submit without printing or emailing anything.

Passwordless sign-in (email code)

Sign in with a one-time code sent to your email — no password to remember. Codes expire shortly for your protection.

Choose how your sign-in code reaches you

Have your one-time sign-in code sent to your email or your phone, and turn on an email alert when your account is signed in to — both under Settings → Sign-in & security.

Login alerts & auto sign-out

New

You're emailed whenever your account is accessed, and the portal signs you out automatically after about 30 minutes of inactivity (with a warning) to protect your health information.

You control the AI features that read your record

New

The AI features that work with your record — the wellness snapshot refresh, medication and plan explanations, visit summaries — need your consent, which you grant or withdraw any time in Settings → AI & privacy. The self-guided worksheet builder works only from what you type into it. Nothing you turn off affects the rest of your portal.
